REFERRAL CRITERIA

Applicants must be aged 18-65 years with dual diagnosis and enduring mental health needs.

RFERRAL BY A SOCIAL WORKER/SERVICE PURCHASER

PREADMISSION ASSESSMENT

Manager meets with applicant within his/ her own environment.
Preadmission assessment report is sent to Service purchaser with 48 hours
Visit to home by applicant is arranged and agreed (Weekend/overnight)
